ADDENDUM / BID BULLETIN NO. 1
CONTRACT TITLE: Janitorial Services for PRA Properties (GOODS 09-009)
This Addendum / Bid Bulletin No. 1 is issued to modify or amend items in the Bid Documents. This shall form an integral part of the Bid Documents.
A. Project ID Number for the Contract: GOODS 09-009
B. Annex A of the Bid Data Sheet (BDS), Page 37:
Technical (Class “A”) Documents
No. 1: Copies of any of the following Contracts, Notice of Award (NOA) or Notice to Proceed (NTP), shall be submitted as attachment or supporting documents for the list of all on-going government and private contracts and statement of all government and private contracts similar in nature as required in SF-Good-13a and 13b in pages 87 and 88. For completed projects, copies of Contracts shall be submitted as attachments.
All the above attachments must be submitted only in one (1) complete set and included in the Original Copy Set of Bid Documents.
Financial (Class “A”) Documents
No. 1: A Credit Line Certificate submitted without a machine validation of the committed amount from the bank will be accepted, however, it should be duly notarized.
No. 3: Should a bidder fail to pay the required Income and Business Tax Returns through the Electronic Filing and Payment System (EFPS) due to technical problems from the BIR or the Banks, a corresponding certification or issuance from the BIR and/or Bank shall be submitted.
Technical Proposal (Contents of Envelope 2)
No. 5: This shall be read as, Schedule of Requirement, Page 63.
C. Section VI, Schedule of Requirements, Page 63:
No. 3: Materials and Supplies, Dipper (Tabo): The quantity shall be reduced to 25 pcs. All materials and supplies shall be delivered on a monthly basis.
D. Section VII, Technical Specifications, Page 66:
No. 4: Cleaning Equipment: 2 pcs – Floor Polishers, 1 pc. – Vacuum Cleaners/Carpet Cleaners.
E. As standard basis and guide for all prospective bidders for the computation of the salaries / wages, the number of days in a year to be considered shall be 313 days, in compliance with the Minimum Wage Law pursuant to Wage Order No. NCR – 14.
